Register Online for TEAM Day 2012

Register online to reserve your class choices and then mail payment ($35 registration from 2/15/12 to 2/28/12) with check payable to Michigan District SDMI payment to:  TEAM Day, c/o John W. Harris, PO Box 678, Perry, MI  48872

SCHEDULE UPDATES:
  • Lunch Codes for High School on page 4 should read BLH, CLH, and DLH  (BLM, CLM, and DLM are codes for the middle school lunchroom.)
  • Workshop 332 Local Missions Trips with a Lasting Impact, will only be held in the C332 session (not B).
  • Building Bridges with your Muslim Neighbors is scheduled one 1 time in Session C (the booklet states 2).
  • E569 MissioLife: Spiritual Formation Through the Small Group has been moved to Session C and is now C569.
  • A682 The Parent Journey Series will only be held one time instead of 2.  E682 has been canceled.

Cancellations/Refunds:  Cancellations received by phone, email or postmarked no later than February 28, 2012 are non-refundable but transferable to another registrant.  Notification must be given to John or Darlene Harris with the name and workshop preferences of the replacement no later than February 28, 2012.  Registration from transfers will not be received after February 28, 2012.

Walk-in Policy—Due to pre-determined allocation of space, faculty and materials/supplies, individuals who have not registered by the required deadlines, yet arrive at Perry High or Middle Schools expecting to attend TEAM Day 2012, will be unable to participate.
